Students must complete a PAPER S.25B Request for Course Change Form

Visit the Guidance Office in person and pick up a PAPER S.25B Request for Course Change Form (email/phone requests will NOT be accepted).
Have a parent or legal guardian SIGN your COMPLETED form.
